LONDON (AFP) – An independent probe condemned cross-Channel train operator Eurostar Friday for failing to prepare sufficiently for winter weather, which led to breakdowns and mass disruption in December.

Eurostar also had "no plan in place" to deal with the chaos created when five trains carrying more than 2,000 passengers broke down in the Channel Tunnel in the busy pre-Christmas period, the investigation said.

WASHINGTON - A six-car red line train has derailed near the Farragut North station in downtown Washington, Metro officials said.

Spokeswoman Bessy Guevara said there were no reported injuries. Guevara said a preliminary report showed the front wheels of the train's first car slipped off just after 10 a.m. Friday.

Authorities were at the scene trying to get passengers off the train, Guevara said.

Police blocked K Street near the station, while emergency vehicles were converging from all directions.

WASHINGTON -- Two Metro workers were struck and killed by a maintenance vehicle along a track Tuesday, the latest in a string of fatalities since last year in the Washington area's transit system.

The men were on a track when they were hit a few blocks from the Rockville Metro Station, Montgomery County Fire and Rescue spokesman Capt. Oscar Garcia said. One worker died at the scene and the other on the way to a hospital, Metro officials said.

LIMA, Peru-Heavy rains and mudslides in Peru have blocked the train route to the ancient Inca citadel of Machu Picchu, stranding some 2,000 tourists.

Perurail spokeswoman Soledad Caparo says the train operator suspended service Monday and is working to clear rock and mud covering the tracks.

Rail is the only means of transportation on the last leg of the trip to Machu Picchu from the city of Cuzco.

Tourism Minister Martin Perez says the government may send helicopters to evacuate tourists and locals from Machu Picchu Pueblo, the town next to the citadel.

(CNN) -- A six-month-old baby had a miraculous escape after his stroller rolled off a railway station platform and into the path of an incoming train.

The incident, which happened Thursday at Ashburton railway station in Melbourne, Australia, was captured by CCTV.

The footage shows the baby's mother momentarily letting go of the stroller, which then rolls over the edge of the platform. Second later, a train pulls in to the station.

ORLANDO, Fla. - Two monorail trains crashed early Sunday morning in the Magic Kingdom section of Walt Disney World, killing one train's operator, emergency officials said.

The monorail operator died at the scene of the crash, which happened around 2 a.m., said Bo Jones, deputy chief for Reedy Creek Fire Department. The other train operator was not injured, but was taken to a hospital because he was emotionally shaken. Five park guests were treated at the scene.
